We’re looking for a Patient Insight Coordinator who will be the lifeline for 130-180 patients, ensuring they receive timely, high-quality care while avoiding unnecessary hospitalizations. Through weekly check-ins, remote health monitoring, and seamless coordination with providers, you’ll be the bridge between patients and the care they need, all from the comfort of your home.

What You’ll Do:

Champion patient well-being: Check in weekly with patients to assess their health and offer support.

Monitor health data in real time: Track patient vitals (BP, weight, glucose, pulse ox) and identify trends.

Act fast when needed: Escalate concerns to PCPs, specialists, or Home Health agencies before emergencies arise.

Educate and empower: Teach patients how to use remote monitoring devices effectively.

Coordinate care seamlessly: Work with medical teams to ensure patients get the attention they need.

Stay organized: Document patient interactions and trends in the EHR system.

Who You Are:

Passionate about patient care – You believe healthcare should be proactive, not reactive.

Detail-oriented and organized – You can juggle multiple patients and keep everything on track.

A natural communicator – You build trust with patients over the phone and can clearly relay concerns to providers.

Tech-savvy – You’re comfortable using Microsoft Office, Excel, and EHR systems.

What You’ll Need:

Preferred: CMA, MA, CNA, or HHA

1+ year of experience in a clinical or patient-facing role preferred

Reliable high-speed internet & a HIPAA-compliant workspace

Ability to work independently and stay self-motivated
