Garden Park Care Center is seeking an experienced and detail-oriented Part-Time MDS Coordinator to join our team. This is an on-site position with flexible scheduling options during the week, ideal for someone seeking work-life balance while making a meaningful impact in long-term care.

Responsibilities:

Complete and coordinate accurate and timely Minimum Data Set (MDS) assessments and care plans
Ensure compliance with federal and state regulations
Collaborate with interdisciplinary team members to deliver high-quality resident care
Monitor case mix and assist with reimbursement optimization
Maintain up-to-date knowledge of RAI process and regulatory updates

Qualifications:

Current Ohio RN or LPN license in good standing
Previous MDS 3.0 experience required
Strong attention to detail and excellent communication skills
Ability to work independently and collaboratively in a team environment

Schedule:

Part-time hours
Flexible weekday scheduling
On-site only
